Thank you for so generously giving me your time this morning. I very much appreciated your reassurance that there still is some hope for me!! Attached please find three copies of various test results. One is my first liver function test from the time, last August, when I had been feeling unwell and was showing persistent symptoms for some time. One is my most recent liver function test, from blood samples taken in late January 2013 and finally the recent result showing my current Hep B status. I am sending you this information from my work email address, which is fine for you to use when communicating with me. I will try to contact you by phone later this afternoon when you may have had a chance to look at these figures and give me your thoughts. Once again thanks for your help.

This poor chap had been given very little advice on what HBV blood tests and Liver Function Tests mean so his results where as follows
August 2012 ALT 236 GGT 38 Bilirubin 12
Jan 2013 ALT 154 GGT 42 Bilirubin 18
Basically this patient had an awful diet for someone with HBV of Indian Origin he lived on Fried foods starting at breakfast and with ghee rich curries washed down by alcohol the GP had advised is fine in moderation and painkillers he was prescribed for arthritis. He had replaced some of the alcohol with energy drinks which is famous for aggravating his arthritis and needed more painkillers. Like many patients he ended up worrying why scores were going up and down and had no ability to understand or effect them. The fluctuations in bilirubin and GGT and worried him but he had not realised Firstly all liver functions tests the alt the ast the ggt the bilirubin bounce up and down daily with every meal. The normal range bit is all that matters with these tests so his GGT and bilirubin scores meant very fit and no problem meaning with GGT which indicates liver damage his liver was fine and with bilirubin at normal his digestion was also good. The bad score ALTs he had not understood ALTs measure the liver working hard and 0 to 50 is great but his scores said too hard. We got him off painkillers got his wife to cook zero fried and way less ghee and banned alcohol and fizzy drinks which brought his scores back to normal. Not only did his liver symptoms vanish tired bloated with right quadrant pain but also his arthritis faded hugely.
With his HBV blood tests HBsAg positive HBeAb positive viral load 80,000IU/ml
we were able to point out his HBV was not growing but a low load that should be managed with diet not treatment with anti virals and that he had an e antibody meaning the HBV was not going to increase much to loads of millions. Once his liver had time to work on the HBV instead of eating toxins all day like fried fat ghee and alcohol pain killers and fizzy drinks his load subsided to 25.000IU/ml and his ALTS have been normal range now for 2 years.
It would do so much good if patients with liver disease had a simple leaflet to understand hbv loads and liver blood tests as they can take charge of these things and really I am always amazed at how good patients are at healing themselves with diet do' and don't's when they understand these things.
